Filip, who takes along his best friend and fellow filmmaker/presenter Fredrik Wikingsson, is convinced that the road trip to the Mediterranean will lift his depressed father's spirits by reliving some beautiful moments and memories. The three men leave Filip's mother behind in Sweden and set off in exactly the same orange Renault 4 that the Hammar family used to have. However, things go wrong almost immediately when Lars falls in his hotel room at night and breaks a bone. Since Lars has to stay in the hospital in Malmö until he can walk again, Filip and Fredrik continue the journey through Denmark, Germany, and Belgium, and just before crossing the border into France, they are reunited with Lars. Upon arriving in Beaulieu-sur-Mer, the seaside town that meant so much to Lars, the sea air and beautiful surroundings seem to do him good right away. While the men settle into the same apartment where the Hammar family used to stay, Filip organizes various activities he remembers from his childhood, hoping to encourage his aging father. But Filip eventually begins to realize that his elderly, sick father will never be the man he once knew and is reluctantly forced to accept the inevitable: that he is slowly losing his father.
The Last Journey offers an honest look at the approach of death, through the eyes of a desperate son who has long denied the inevitable, a heartfelt exploration of the end that ultimately awaits us all with our parents, and a glimpse into our own future.
